The confusion was caused by a bug in either cubase or the asio driver, not sure which.

At a sample rate of 44.1k the card has 26 outputs available, but only 14 at high sample rates. When you select a high sample rate cubase still says it's using the SP/Dif output, but check the drop down and that output is no longer available... hence the confusion.
