The key for sight reading at piano, and I expect it works for guitar as well, is not to read "notes" but to read shapes. I am assuming you are reading written out music rather than a lead sheet.

You simply cannot read a vertically stacked D F G B, for example, as individual notes and then find the notes individually. You have to go directly from shape on the page to hand shape. Perhaps you recognize that as G7 at the same time, which is useful if you have the writtenm part but don't want to play the plain notes as given.
