If you've been following will_m's thread on re-attaching a broken string ground you can use a similar method to the one we describe there for pulling the bushing.
The secret is to find a bolt or machine screw where the shaft passes through the ferrule and the head is small enough to pass through the holes in the body but too large to pass through the ferrule. Then improvise something with a socket, some washers, nuts, and padding.
