The form is so the production as a whole can get completion insurance and I think it's fair enough.

The bit about passing on data (only) in the event of a claim is presumably so that they can involve external medical opinion should you claim you can't complete the project because your right arm has fallen off... and is, I think, quite normal. Your personal data is always protected by the data protection act, and only those with a genuine and valid reason to see the data is ever allowed to see it.
